RE: post-op lap-band

I review a diet history, first looking to make sure they are meeting their

protein needs, then look at fiber intake. I ask about constipation vs

diarrhea, most have some constipation due to low fiber intake and so I

discuss fiber sources and how that will improve constipation and also is a

good thing for satiety when eating lower calories. Check fluid intake.

Then I explain to them that prior to their first band adjustment they may

feel that they can eat almost normal portions and not have any problems

tolerating different food and so the types of foods they choose and the

amount of activity they do will be the more important thing for weight loss.

Most of my patients say that they did not notice much change in level of

restriction until they had 2-3 band adjustments so it may be quite awhile

before they are able to get more help from the band. So we may need to talk

about calories or low calorie snacks or other typical weight loss nutrition!

like Benefiber or do you stress high fiber foods like beans, fruits, and

vegs?

I first help them figure out what they can get from fruit, vege, grains,

legumes appear to be really well tolerated so I emphasize those. I do list

both benefiber and flax meal as an option if they are not able to get enough

from food.....but stress the importance of getting it from food. I am

attaching a fiber handout that I put together; feel free to use it!

I focus on diet quality & tolerance in office visits. Always ask if they have N/V or Diarrhea/Constipation. Vomiting is the big sign that they may be eating too fast, not chewing well enough, drinking liquids with solids, or eating too much. Many pts admit that they know what they did to make themselves vomit. I educate them extensively on this prior to Sx.

Secondly, constipation is a big complaint during this time-- diet is low in fiber & it is difficult for them to get in their adequate liquids. I have a good handout on constipation, recommending soluble fibers in diet, as well as the OTC products available. I can't attach it right now, as our computers are havig problems right now.

Our diet advancement post-op is 2 weeks on full liquid, 2 weeks on puree, then advance to soft protein foods. They are all supposed to be sipping on 1-2 proten drinks a day for that first month. I always ask if they are compliant with the drinks...

I also focus on their excercise.... they should have started walking on a regular basis at this time. Those who do not get into an exercise routine at this time usually never get into it!

At week 6, they are eligible for the first adjustment. If so, we tell them 2 days post-adjustment on clear liquids.
