WC guidelines re: bundling charges for 97124-59 with 98940?

Hi Colleagues,Has anybody had their 97124-59 charges denied on a w/c claim on the basis that it's part of the spinal manipulation & /or to the same area as the cmt? The review company from CA says that this is a " national policy " . I've never had an issue before and wonder if there's been a change?

What recourse do we have if a TPA isn't playing by the rules and refuses to make proper payment on a w/c claim?Thanks,
