Guest guest Posted January 14, 2010 Report Share Posted January 14, 2010 Hi Ron, The states 18 visits or 60 days of treatment whichever comes first and as such the patient is already over the 18 visits. As a consequence you are no longer the attending physician and must obtain a referral from a medical physician. BTW we use the Form 827 for " change of physician " pretty sure the Form 829 no longer exists. More importantly if you want further curitive chiropractic treatment via an MD there are some very specific things that must be included in the medical doctors referral something they never understand nor wish to to as a consequence you must do it for them with a place to sign off on it. I have a standard macro that I use that refers to the ORS and has all the needed language and a place for them to sign off and fax the form back to me. It's always best if you can speak to the doc personally on the phone or immediate staff giving them a heads up that your are faxing over your " Worker Comp referal note " for the doc to sign and fax back. Alternatively you can have the patient hand carry the form to their appointment with the medical doctor if that particular doctor needs to see them first but make sure you have the patient take the form back once signed and bring it to you to send in...DON'T depend on the medical office to send in the referral note they always mess stuff up or simply don't do it. I've faxed this W/C referal macro to lots of docs if you or any colleagues reading this post wish to have me fax the form to you give me your fax and I'll send it tomorrow once back in clinc....
