(also, sorry for the cross-posting) we have a 36 year old male - BMI 40 - with type 1 diabetes - is on an insulin pump and is increasingly insulin resistant. Has co-mobidities of mobility, high blood pressure, lack of energy.... and is seeking approval for the Roux-en-Y procedure. My question: has anyone else had a Roux-en-Y patient with type 1 diabetes - and if so, what tips would you have to offer?... If approved, I will work with him during the four day hospital period to determine the dosing with the pump -- my guess will be very low basal... Additionally - the monitoring of ketones would also be key during this time frame. It is very odd to have an obese type one - but the patient states at his diagnosis at the age of 25, the physician stated he was type 1 and has been on insulin since. thanks for any ideas/sharing. Guest guest Posted December 16, 2003 Report Share Posted December 16, 2003 I too was diagnosed at age 25 with Type one, put on insulin and I have been on a pump myself but I was not Type 1 and although I do use insulin now I do not use it all the time. I still see pts all the time that docs admit under the diagosis of DKA but they aren't...no ketones. The pt could have antibody testing but that is expensive. I'd do some more checking.
