Thiamine deficiency in Roux-en-Y?

Good morning everyone.

I wanted to pick your brains on this subject. I am at the University of PA Medical Center and our program performs approximately 500 bariatric surgeries per year. Last week, 2 patients were admitted with neuropathy. After the work-up it was determined they had a thiamine deficiency. As far as I know the patients have no liver problems and are not alcoholics. They are both at least 4 months out. They had no major problems with vomiting or diarrhea.

They are currently receiving vitamin/mineral supplementation via IV in the hospital and will be sent home for 2 weeks with the IV. After that, I recommended they should be supplemented with 10-20mg of thiamine per day.

Has anyone had this experience? With all the cases we've done, I had never seen this before.

Whatever feedback you can provide, would be extremely helpful.

W have had one case of beri beri due to Thiamine deficiency. She was not

compliant with vitamin supplementation and developed it within 3 months

after the roux-en-y surgery. This person didn't have alcohol or liver

problems, either. Unfortunately, we didn't have her baseline Thiamine

levels. We have added pre-op Thiamine level.

Curious if they were taking a MVI for the past 4 months and if they were if it was tablet or chewable or liquid?

Does anyone have any information on homemade feeding formulas for patients that do not want to use processed can items. To be used with J-peg.

I have a patient looking for this.
