Hi, ! Definitely send out small samples!! If your sample size is less than

1 oz, the FDA does not require it to include ingredients. That generally makes

it possible to use the very tiny address labels to label the containers. It's

also nice to put samples in a 3 x 4 " ziploc bag that can easily have a label

attached.

I guess neither of those ideas is perfect, but the return you get from

distributing samples is DEFINITELY worth any extra effort needed to figure out

how to package and label the samples! :)

I also give the patients some samples at the pre-op consult. I have Unjury

and also Juven (available in grape and orange flavor) - contact the Ross rep

nearest you and ask for some samples of Juven. Bariatric Advantage sent me

a big box of samples recently (chewable Ca and Iron, liquid Fiber, protein

shakes) so I will give some of those out too.

I include splenda, sweet n'low, & equal as well, especially if the

patients have never used these before. Nestle will send you samples

of No sugar added CIB as well. I have included opitsource products

before, and Nectar Protein supplements as well. Unjury is great

about samples.

Thanks,

I've never recorded them. If we're supposed to be doing that, then I hope someone else comments on this!

Thanks for the question,

From: Beth Taschuk <bethhanne@...>Subject: samples Date: Thursday, April 30, 2009, 10:02 AM

Good morning:

It is my understanding that it is not required by JCAHO to document samples(such as protein samples) distributed to patients. Am I correct or do we need to record patient name, expiration date, lot number, etc?..

Thank you.

I want to thank all the vendors whom donated for the 2009 cny gathering.

I do however need help with two of them.

One is from " go planetearth " I've got a tube of fragrance oil here with no label

on the fragrance.

the other is a small dram sniffie I think from save on scents, it is orange

color fragrance. Once again, no label.

Help, can anyone help me out with the name of these fragrances?

Thanks,

Cheryl
