My son has been on 10 mg Paxil for many months now quite successfully. He

recently started his OCD behaviors again (tapping, chewing his fingers, etc)

so we increased it to 15 mg daily. It hasn't helped and the symptoms seem to

be increasing. May I ask is 20 mg a safe dose for an 8 year old? Does Paxil

stop working suddenly? Anyone have any insight? Thanks..

Hi ,

When a child who has been responding positively to a medication experiences

a setback it may simply be a waxing of the OCD that may pass or it may

indicate the necessity for an increase in dosage. I have cc'd your post to

Dr. Dan Geller, our resident physician on the list. Hopefully, he will

respond.

Have you subscribed to the OCD-L. Dr. Jenike has helped me on

several occassions with suggestions for changing medications for our older

son who has OCD. Dr. Jenike's suggestions have resulted in a real

improvemnet in the quality of our son's life. If you subscribe to the

OCD-L just post a message with Dr. Jenike's name in the subject line and he

will respond.

HI Gail:

IN general SSRIs are considered " safe " medications. Of course there are no

long term studies for kids on Paxil so we are basically experimenting when

they are taking Paxil off-label. My son, Steve, has been on Paxil for

nineteen months, he was on 60 mg when he was 10 and now he is 12 he is on

20 mg and wants to titrate down further again. At first I was very worried

about his taking meds, however for his depression Paxil has literally been

a lifesaver. This has really changed my attitude as I have seen what it

has done for him. He is much more alert on 20 mg than the sleepy kid he

often was on 60 mg.

Medications need to be adjusted as our kids grow older, since they may need

a higher dosage for their increasing body weight to get the same effect.

Occasionally it seems the meds can stop working but from what I have read

this is not typical. Steve was able to reduce his dosage because he

started doing very well with his CBT. If you check out the OCF web site

page on OCD meds for children you will see that kids often take the same

dosages as adults because they metabolize the meds so rapidly and SSRIs

often are active for a short time in their bodies.
