The thing is, he might be able to change enough to make it all more reasonable. Like, I don't think we could fully privatize social security, but why not a portion? I was looking at a statement for someone who, over the course of his work life (he's 40) has paid in $55,000 in FICA, and his employers have paid in $55k, for a total of $110,000 so far. When I was taking economics we were shown a graph that if $1,000 was invested and nothing else added, and interest was accrued over so many decades, I can't remember for sure, like 40-50 yrs, following the yearly average of the stock market through history, even through the Great Depression, the person would have nearly 2 million dollars. So why not take $1,000 out of the $14,000 paid in yearly for working Americans and allow them to invest it privately? Same with health savings accounts. I think right now we're limited to something like $5,250/family/year that can be put aside for medical expense. Why not more? If a family can set aside $10k/yr, then why not? Sure, that would be used in a day in ICU, but the point is to grow it while keeping catastrophic coverage. For my healthy girls, if I could put back just $1k/yrly into an accruing medical savings at birth, without penalty or taxes, in 18 -20 yrs she could have a nice nest egg for health-related issues, while only carrying a catastrophic policy. Sure, for our kids with ASD it's near impossible, but it's a start to lower the cost of health care. I'm telling you from personal experience, when we pay for our own care instead of thinking a health insurance co magically pays for everything, health insurance will go down. Do I think can/should go as radical as he desires? No, I don't think it's practical, despite it sounding good on paper. But that's what Washington needs, someone extreme enough to bring about change. No one who is part of the establishment, be it , Clinton, Romney, or McCain can change anything, they're too much like the rest. Competency vs Continuing Education is the national trend for health care professionals. Oregon has one of the broadest scopes of practice in the country, and in order to ensure that our scope is maintained and properly " serviced " , it is my personal opinion that we would benefit from stronger Continuing Competency practices. Peer contact does, to some extent, encourage and revitalize professional interest, and stimulates constructive dialogue. Thus, it furthers acquired knowledge base (vs video hours, for example, which can be quite instructive for the highly motivated DC, but often are solely utilized to fulfill the hours requirement as inexpensively as possible, or as expediently as possible if the doctor has waited until the last minute. In these cases, absent a test at the end, the video courses may require little actual engagement by the DC.) In the bigger picture, (in my opinion), one of the best ways of ensuring enthusiasm and a healthy practice is to maintain one's professional competency by staying abreast of the latest techniques/research/practice parameters in the DC's area of personal interest. Live seminars MAY incur extra cost for remote rural practitioners, or for those who don¹t plan ahead well. For example (not that Bend is remote, but it is a distance that easily comes to mind for comparison purposes): Hotel room (inexpensive) $79 Fuel $53 ------------------------------------ Total $134* *Actual results may vary. Based on: $79/night for an inexpensive hotel room IF the person had to travel and didn't have a colleague or friend that they wanted to stay with. Gas money- say a round trip from Portland to Bend @ 150 miles '/. 20 mpg =7.5 gals x $3.50/gal =$26 x2 (RT) = $53. Of course the cost of the education itself is a constant, no matter what one chooses: some seminars are cheap or free, some are quite expensive. For example, Dr. Guerrero reminded me of a group that Dr. Lee Cowan started in the East County (Gresham area) which has been meeting monthly for some time: each month a DC presents a case to the group and there is discussion. I think this sort of meeting would constitute valid " live " education at almost no cost. Yet it would engage the mind of participating DCs and foster peer interaction. This is completely 'non-denominational.' Another option might be to volunteer for participation on Board committees. There is continuing education credit for some of these active participation hours There may also be opportunities to proctor continuing ed classes offered at WSCC or for traveling seminars that are available at reduced or no cost to the doctor willing to sit at the door and check people in and out. Perhaps if there is a real difficulty with $$, and an association want to support their members, a needs-based scholarship would be established, OR maybe the Board wouldn't require the hours to be in person until a DCs third year in practice... I¹m sure there could be other possible solutions out there. I'm quite certain that if the proposed rule passes, that many more live seminar opportunities will arise. Let me also be clear that while the Board was pleased to learn that the state associations were interested in this idea, it was completely independent of the Board's initial discussion and direction. And, of course, the Board would not influence the profession to attend any particular seminar, and in fact, these could (as now) be taken out of state. It would be a shame if an opportunity for improvement was lost due to the completely erroneous perception that the OBCE was fostering this change to suit the state association(s). Nothing could be further from the truth. Having said that, I DO think it is wonderful to have open dialogue between the OBCE, the state association(s) and individual DCs, as well as any of our other stakeholders. I believe this serves all of the OBCE stakeholders. Lastly, to address concerns of some re: the ³timing² vs. current economy, perhaps this is something that we might set to start one year from now. This would allow our doctors as well as those who deliver continuing education a little lead time to ease the transition.
