Guest guest
Good morning, I just saw this patient again. He has improved DRASTICALLY. He claims to be experiencing no pain since he stopped drinking, started taking mastic gum, slippery elm and antacid once a day or so. He started feeling better on the 2nd day. On the 3rd day he decided to indulge in "a few" pints and woke up the next day miserable again. He now understands the importance of avoiding alcohol at least until this is healed up and was in good spirits and pain free this morning. He also conveyed to me a clearer pattern of the pain profile. Seems to correlate very closely with that of duodenal ulcer. Pain 2hrs after eating etc. In a few days he is instructed to go with out the antacid and observe the results. Hopefully get some healing of the ulcer so that the acid will not irritate. Never knew beer could irritate an ulcer so badly (thats all he drinks) . Just relieved that the conservative approach and diagnosis was prudent. Not to chase white elephants without substantial red flags or reasoning was a lesson learned. Thanks again to all who contributed to the thread. No smoking, good diet, exercises, but admits to frequent alcohol consumption. He's complaining of intermittant upper abdominal pain or discomfort (mild aching gnawing type of pain) The pain is located in the upper right quadrant just below rib cage 4-6 inches from midline. The pain occurs in waves and increased under stress, but has no correlation with eating, or waking or anything that would be consistant with ulcer. No diahrrea etc. all bowel function appears normal. At times he experiences the pain after excercise. Antacids provide no relief.. Theres no urinary problems, no rebound tenderness etc. The pain is worse sometimes when he drinks heavily, but sometimes it's not. I'm bewildered, any thoughts????
