Basically, my communication sheets were tables made with MS Word that included various categories and then items or ratings that could be checked off in each category, as well as a place for comments in each category's box. For example, for lunch you could have " menu, ate well, ate poorly " ; for math you could have " worked on ___ objective, good progress, still trying, inattentive, mastered ____ " . Only you know what you really want to hear about. You can brainstorm some categories and go from there. You might think in terms of special events, therapies, self-help (could include bathroom, tying shoes...), math, language/reading, behavior issues, etc. Give yourself a few days, because more ideas will come to you after you get started. Guest guest Posted August 11, 2005 Report Share Posted August 11, 2005 Stefanie - We've actually eliminated the " written " notebook and have gone to daily e-mails...this way there is always a copy (and no one can make changes)...plus the teachers seem to like this because then they can make notes during the day and then when school is over and they are finishing up for the day they can write something at a more relaxed time for them...makes them feel less pushed... But, lifted from other threads on other boards, here are some possibilities: His teacher (who is no longer at his school for me to ask) found the communication sheets that she used in a book. They are pecs-like pictures that are 1 1/4 inches by 1 1/4 inches. The title is: I Like Me! My Day At School Today at school, I (.....followed by the pictures) At the bottom, it has Special Notes: (and a space to write info in) From: (and it is signed by whomever wrote the notes and assisted with the sheet) The school pictures were, Read a Book, Went to Library, Used the Computer, Cooked, Did Puzzles, Played Outside, Played with Toys, Ate, Had Music, Had Art, Had PE. There was also a bland square that she filled in for things like PT, speech, math, etc. She also drew some more blanks off to the side and filled in as necessary. The home sheet had Ate Dinner, Took a Bath, Read a Book, Cooked, Watched TV, Played Outside, Played with toys, Saw Friends, Used the Computer, Saw a Movie, Went Shopping. Before she started the above sheets, she used another sheet as follows. This sheet only took minutes to fill out because she mostly used checkmarks. She would fill in the blanks for some items such as cut w/scissors for Fine Motor Skills, etc. Here goes, I hope its not too long. LOOK WHAT I DID TODAY!
