I have a patient with a large paracentral

disc extrusion at L5-S1 and broad-based protrusion resulting in Left S1 nerve

root entrapment, along with moderately severe stenosis of the neural foramina

bilaterally. I will be referring

for a surgical consult, patient requested to be sent to NW Neurosurgical

Associates to see Dr Soldevilla.

Looking for a

recommendation regarding NW Neurosurgical Associates, Dr Soldevilla, MD for a

Lumbar Disc patient.
