To All,

I was recently made aware of this tier system within ASHN. The rating on " %

of submitted treatment plans accepted " stuck out to me. When I was a

student at WSCC a few years ago, we were TAUGHT that when dealing with ASHN

in the outpatient clinic, we should always request twice the number of

visits you needed and twice the number of modalities you used. The

rationale was that they routinely halved whatever you put down. I have been

assured by other classmates that they received no such instruction, so at

least what I learned does not appear to be endemic there. Perhaps it sheds

some light on the practice habits of many clinicians, and points out an area

of questionable ethics that should be addressed at the teaching level.

I have been a Chironet member for 10 + years and A Ashn member for 2+ years. Chironet used to provide their providers with a feedback on how they were doing with regard to utilization, costs etc. I was always in the middle of the pack or on the thrifty side. ASHN just did a review under this you get 12 visits if you are good rule and guess what I wasn't good enough, I'm still at the 5 visit level. I t looks like the 12 visit thing is an unreachable carrot. I only joined ASHN as a courtesy to my younger associates( All Docs in a clinic must be on the panel for any to participate). Yea, they actully have this rediculous grading system in place. I remember receiving my "grade", which was not in the 90 percentile. It's ironic and confusing and my guess is that nobody is in that %. It's much more complicated than just 90% approval. Docs that have 90% approval aren't asking for many visits and there is little doubt that this is just another way to increase their bottom line.

You know, what ultimately galls me with all of this is Do they really have

that much disregard for their subscribers ... do they think we (DCs) are all

country bumpkins just off the turnip truck? They must think we are all

sheep, just braying in line on our way to feed trough .. that is less than

1/4 full! It truly is galling.

sorry .... just one of those moments when I shake my head at the stupidity

of our health (???) care system.

As honest,

hard working, and caring professionals, we often ask these questions about

managed care whether it is the home grown exploiters like CHP or the big guys

like ASHN, as if we were dealing with the same sort of people that most of us

are. Here’s a little fable that sums it all up for me as far as these

managed care gougers are concerned:

The

Scorpion and the Frog

One day, a

scorpion looked around at the mountain where he lived and decided that he

wanted a change. So he set out on a journey through the forests and hills. He

climbed over rocks and under vines and kept going until he reached a river.

The river

was wide and swift, and the scorpion stopped to reconsider the situation. He

couldn't see any way across. So he ran upriver and then checked downriver, all

the while thinking that he might have to turn back.

Suddenly,

he saw a frog sitting in the rushes by the bank of the stream on the other side

of the river. He decided to ask the frog for help getting across the stream.

" Hellooo

Mr. Frog! " called the scorpion across the water, " Would you be so

kind as to give me a ride on your back across the river? "

" Well

now, Mr. Scorpion! How do I know that if I try to help you, you wont try to

kill me? " asked the frog hesitantly.

" Because, "

the scorpion replied, " If I try to kill you, then I would die too, for you

see I cannot swim! "

Now this

seemed to make sense to the frog. But he asked. " What about when I get

close to the bank? You could still try to kill me and get back to the

shore! "

" This

is true, " agreed the scorpion, " But then I wouldn't be able to get to

the other side of the river! "

" Alright

then...how do I know you wont just wait till we get to the other side and THEN

kill me? " said the frog.

" Ahh..., "

crooned the scorpion, " Because you see, once you've taken me to the other

side of this river, I will be so grateful for your help, that it would hardly

be fair to reward you with death, now would it?! "

So the frog

agreed to take the scorpion across the river. He swam over to the bank and

settled himself near the mud to pick up his passenger. The scorpion crawled

onto the frog's back, his sharp claws prickling into the frog's soft hide, and

the frog slid into the river. The muddy water swirled around them, but the frog

stayed near the surface so the scorpion would not drown. He kicked strongly

through the first half of the stream, his flippers paddling wildly against the

current.

Halfway

across the river, the frog suddenly felt a sharp sting in his back and, out of

the corner of his eye, saw the scorpion remove his stinger from the frog's

back. A deadening numbness began to creep into his limbs.

" You

fool! " croaked the frog, " Now we shall both die! Why on earth did you

do that? "

The

scorpion shrugged, and did a little jig on the drownings frog's back.

" I

could not help myself. It is my nature. "

But your fable is painfully close to the truth. The saddest mystery is why so

many of these well intentioned chiropractors still join these HMO's in spite of

sensing the truth of your fable.

I can only speak for myself having joined a number of these many years ago.

There are three reasons I saw to join. One was that I recognized that

patients and people I hoped would become patients had health insurance that

included this crappy HMO coverage for chiropractic and some of them

verbalized that their decision on whether to get chiropractic care and who

to get it from was largely determined by which provider was covered by their

plan, so I figured I needed the HMOs to have a busy practice. The second

was professional low self-esteem; the absence of confidence that I had

something of such value that the patient would elect to come to me even if

their insurance didn't cover my services. The third was remedial level math

skills. I actually thought that by being paid what the contracts stipulated

I would be paid that I would get by and by growing the patient base with

referrals from such fiscally marginal patients of their better insured

friends that I would do alright.

I was wrong on all counts and a few years ago I wrote a number of scathing

posts (which were a delight to write, I gotta tell you) targeting the

" Pirates of Managed Care " . That even drew a threatening email and phone

call from an attorney at ACN, who demanded an apology. I never wrote the

apology, because I'm still waiting for the personal service I told him such

apology would be contingent upon.

I was wrong about the math because these crooks in managed care screw the

provider every way they can. On the surface, my math said I'd just get by

providing for those managed care patients. In reality, when the managed

care cons lost billings, I had to catch the mistake and rebill within a very

short window of time or forfeit even the tiny reimbursement I thought would

be thrown my way. They have a whole battery of such ploys, and just like

the electronics and software vendors who offer rebates on product rather

than a straight discount because they know many people won't dot every " i "

that it takes to get the rebate, the managed care banditos know that in the

real world of chiropractic offices there will be a significant percentage of

billings that they can get to fall through the loopholes they have designed

to cheat you. So, what was originally going to be marginal business

actually costs you money.

I finally realized that my services were not just worth what I charge, but

that they were a real bargain in the health care marketplace. I have

patients making decisions about my care just like anything else they budget

for. They recognize the value and prioritize according to need. Often,

they decide to put off that set of 20 " spinning rims with glowing LEDs for

the SUV and get the chiropractic care they need. Many patients now discuss

with me shopping for health insurance based on how well they are reimbursed

for the chiropractic care they already know they are going to purchase from

me. The crappy insurance company is what they recognize to be expendable,

not quality chiropractic services.

So, I am all for the strategy of taking away from them the only thing of

value that they have to sell, which is the panel of competent providers our

collusion with them provides them. Fiscal salvation for the chiropractic

office will never come by working through managed care, but by working

around it.

And remember, when you place the last letter of HMOs at the beginning, you

get shmo. Verrrrrry interesting.

I just read this post (for some reason I didn't see it when it was posted),

but I had to laugh when I read:

That even drew a threatening email and phone

call from an attorney at ACN, who demanded an apology. I never wrote the

apology, because I'm still waiting for the personal service I told him such

apology would be contingent upon.
