Usually, patients have been screened, and have had provocative discography and diagnostic/therapeutic epidural injections to help identify the pain generators. In my experience, it has usually been suggested as a last ditch effort.....when it doesn't seem surgical. Of course, that was up until now. Today, had a patient who it was suggested to DURING the second round of injections. Was kind of surpised that the physician didn't wait to see how the second injection was going to work!!!!!! Made me suspicious of it being a "business" thing. I've only had two patients who have had the procedure, and it definitely improved the quality of their lives. For some of them, to have the promise of 12 to 18 months of reduction of pain before the re-growth of the nerve and the peripheral sprouting and reoccurence of pain ....is a blessing. THey both used Dr. Karaseck in Eugene. Locally, Dr. Roy Slack has done the discography. Hey , does Dr. Tiley do discography?

Mike Kurosec here in Eugene does the IDET but I haven't heard anything about the outcomes. Sunny ;'-)

I have had three patients IDETed. One was great, another failed once but was better after the second procedure. She eventually improved significantly after a year........ passage of time? Another had no change. The idea is good but I think patient identification for the procedure is not that great. Just the presence of a fissure in a disc does not mean that it is causing the pain..............

I sent one patient to Dr. Kurosec about 18 months ago; good reports etc but the procedure was not successful....... :0( so the pt continues w/sciatic pain the heel.........IN SPITE of various chiro care.....and she DOES have an odd disc........

This business of spotty results with IDET reminds me that we should all be aware of the variety of chiropractic techniques out there. If your diversified hasn't produced results, send em to Activator before an MD. If SOT hasn't worked, send em to Gonstead, etc. We owe it to our brothers and sisters to seek out the variety that this grand profession has to offer...Ron Johansen DC, SE Portland

Ron your point is very valid and I support

it. The patient who needs this referral is a Work Comp patient. She

saw me on a consultative basis but cannot see me for ongoing treatment. Her

Ortho Neuro McKenzie exam indicates a disrupted disc with derangement and

chemical pain generators (peripheral disc fragmentation). She has been

treated for over a year, seen every kind of specialist (after prolonged referral

time) under the sun and had every diagnostic test under the sun except

discography. She is captive to the Work Comp beast but her attending specialist

will consider a discography referral. If the test is positive, then IDET

(since she has had epidural) is a logical progression. I agree that if

the exam indicates mechanical pain generators (Ortho or Neuro subluxation) and

your treatment protocols are insufficient then by all means refer in a timely

fashion for alternate therapeutic trial. Our reputations are built on our

successes and we are still perceived as successful when we make a timely referral

and the patient gets the help they need.

Larry I use Lloyd Hiebert, MD in Salem, Oregon for provocative discography, I have yet to send a patient on for IDET but he performs this procedure as well.

Dr. Hiebert can be reached at the Northbank Surgical Center at 503-371-4647.

Thanks , I've never used Olson as of yet but if he is associated with you he must be tops!

Don't know about the folks were Dr. Hiebert is performing his procedures but Lloyd has been nothing short of tops as well, a strong plaintiff advocate, a damned nice guy, and get this when I asked last election cycle he gave our Chiropracic political action PAC (ICE-PAC) $1000.00 !!

While I definitely applaud Lloyd's support of the profession, it's hard to ignore the fact that the facility that he works at is so openly hostile to chiropractors.

Don't know about the folks were Dr. Hiebert is performing his procedures but Lloyd has been nothing short of tops as well, a strong plaintiff advocate, a damned nice guy, and get this when I asked last election cycle he gave our Chiropracic political action PAC (ICE-PAC) $1000.00 !!
