I have my 6-month follow-up in a couple weeks. You don't have your labs

done by then, I'll stop by and introduce you to my Size 9 5E steel toed

boots on my way through to Delano. (I'd do it sooner but we'll be back in

the mid-West for the next couple weeks.) Enough incentive for you? LOL

Pat,

This would be a good reason for me to get the last two flights in that I

need for a free trip on Southwest. We get Lee to have her labs done

and I get a free flight out of it. ;-))

My brain is foggy from this blasted cold so I'm going to kick your

tiny little hiney right now rather than later because I'll probably

forget. You HAVE to get your labs done!! This is a matter of LIFE

AND DEATH!!! This is nothing to screw around with! It takes no

more than 5 minutes to have your blood drawn and when I had mine

done a month ago, it was painless. You could DIE if you have a

deficiency and don't catch it in time! Please, Lee, do NOT

mess around with this! Get your labs done, sweetie! Yesterday!!!

It works wonders for me, especially when I'm not 100% into the

program mentally! Here's today's menu for me...and I'll fess up

that I don't have veggies on my plate today but promise to eat them

tomorrow :) I am going shopping after work.

Turkey, mashed potatoes

Turkey, apple

CC, 1/2 serving yogurt, 1/2 serving blackberries

CC, 1/2 serving yogurt, 1/2 serving blackberries

Bar (BOO HISS...should have planned better but I have pt and spaced

Meal 3: EAS Low Carb MRB and 1/2 side salad from Chick-Fil-A

Meal 4: CC Shake (1/2 cup lowfat CC, 1/2 cup FF Yogurt, 1/2 cup Pink

Lemonade Crystal Light, 1/4 cup Mixed Berries)

Meal 5: Will be 3 oz chicken on a WW Tortilla with FF Cheese and FF Ranch

Dressing and the rest of the side salad

Meal 6: Will be 1/2 cup oatmeal with 1 serving of tofu and 2 oz of turkey

Here are my eats for the day:

Nutrition:

1: Chocolate banana shake

2: CC pancakes and veggies

3: Chocolate banana shake

4: Oatmeal tuna snack with apple

5: 3x1 veggie omelette

6: Chocolate strawberry/apple protein cupcakes (Sara Lake's Choco Cake recipe)

Supplements:

Multivitamin, Calcium 500+D, ~2 T. safflower oil (in shakes and used

in cooking meal 5)

Calories eaten and deficit for yesterday = 1502 / -346 (target -437)

Here's my food today.

meal one (post work-out) One Way Bar

meal 2 salad my mom made w/ tuna fish, lettuce tomatoes, feta cheese

and some grapes. I am not even sure what kind of dressing she used.

(I am usually stricer with my meals but was at parents and this was

better than the Burger King meal my kids were eating)

meal 3 slice of Brownberry Carb Control bread and small can of tuna

meal 4 4 or 5 oz of chicken breast, 1/2 cup oatmeal and brocolli

Meal 5 1/2 cup oatmeal and ff cottage cheese

have not had meal 6 - If I have anything will be protein shake w/ milk

Exercising: did HIIT on Eliptical and some leg and ab exercises.

work-out cut short because I had to take son to restroom a couple

times.

I just found out this week that my gym is closing sept 15. I am

pretty bummed about it. Luckily I just went back to graduate school

and will be able to use the gym at my school. I will not be able to

bring my kids, so it will take more work to arrange my work-outs

around my husband's schedule. I am more sad about the fact that I

will miss the friends I have made at my gym.

I know I'm the one who suggested it, but... I think I'll get back to the

accountability next week. I just got my Betagen & Thremodynamix from Vitaglo

(glad that package came!) but I think I'll wait 'til I take my 4 week photos

to start taking it again. Not sure if there will be any difference, since I

haven't been eating clean, but...

Hi ,

Good call on staying on plan for the rest of the day despite this

morning's set backs. Here's a portion of a message I posted to

another board in response to a member's " I'm unmotivated because I'm

afraid to fail " excuse for scrapping the entire week after one

unauthorized nacho snack on Monday:

What I'm fighting right now is my perfectionist tendency, that part of

me that wants to say OK YOU SCREWED UP THE *ENTIRE* DAY, YOU ARE A LOSER

anyway). I am not a stupid person, but I persist in holding on to these

attitudes that really are stupid! Of course you wouldn't slash all your

tires! You'd fix what was wrong and move on. Why is it so difficult to

apply that same reasoning to the rest of our lives?

It is because we can't get over that we had a flat tire in the first

place and then we mentally use it as a scape goat for what we really

to do, eat bad. Instead of bringing the tire to the junk yard and

focusing on our shiny new tire, the new me...

Yesterday was not that good in the evening:

1/2 cup oatmeal, 4 egg white and 1 whole egg

Veggie burger, ff cheese and 1 apple

Dexatrim All In One MRB

CC and Kashi Mix (posted recipe earlier)

Chicken Fajita and 1/2 bowl of Fat Free Jell-O Pudding (OOPPS!)

The other half of the Fat Free Jell-O Pudding (Double OOPPS!)

Good Points:

Still didn't eat the Oreo's.

Bad Points:

Ate a whole box of FF Jell-O Pudding. I really had good intentions and made

it for my daughter. At least it was ff milk and I did get some protein

from it.

Missed my cardio because I had two many conflicts during the day. First

missed workout since I started 4 weeks ago. I did it this morning though

and am caught up now. :o)

I think part of what happens to me when I get off track is that I have no

clue how much what I've eaten will impact my daily total. (Since I utterly

refuse to count calories...)

So once I have a slice of pizza or a donut... I give up on eating clean for

the whole day. I know that's stupid...

Accountability

Well, after my splendid start yesterday, I ran right into my brick wall

today. Even though I went to bed on time yesterday, I could NOT get out of

bed this morning. Got into work late. I've had one meal - whole wheat

toast with breakfast patties plus one string cheese. I did not pack up the

tower of food.

What I'm fighting right now is my perfectionist tendency, that part of me

OK, . It is 10:27 AM. The *entire* day is not ruined. I remember

someone on this list once said you are never more than two hours away from

being back on track. I will not be able to fit in a workout today, but I

can eat right the rest of the day, I can get in my water, and I can be

nice to myself.

I know that was sort of stream of consciousness rambling, but it makes me

accountable to myself if I tell y'all my plans.

For future reference:

Slice of pizza = 250-400 calories, mostly carb and fat depending on

what kind you have

Glazed regular doughnut ~ 200 calories, again mostly carbs and fat

Cake doughnuts ~ 250+ calories

So if you have slice of pizza, you can pretty much count that as

sucking up the calories that should have gone to TWO BFL meals. A

doughnut...maybe just one.

;)

I find it best just to have a variety of authorized foods on hand that

are taste substitutes for the bad foods I normally crave, including

those microwave muffins (good doughnut substitute), BFL chocolate

mousse, and frozen BFL chicken pita pizzas. That way I KNOW each one

is 200-275 calories and balanced.

And if I MUST have some M & Ms, I make sure I just grab a set number

like 5 of them, and once they finished, I don't have any more. I

can't let myself just grab one or two every time I pass the candy dish

or it's all over.

Hi lydia, Don't beat yourself up. Cut yourself some slack, but a little bfl is

better

than none! I'm hearing you that you don't want to count calories, but I was

curious....:)

I've been known to stop by krispy kreme on a free day ..... So I wanted to know

just

how bad they are. after checking out their nutritional info, - of course they

are just

pure crap, but at least, I know just how bad they are and realized that if I

just kept it

to one " original glazed " its much better than some of those creme filled ones,

(and a

TON better than having say THREE......) plus then I realized I should maybe have

that

glass of milk with it for the proteins sake! (bad jojo!)

original glazed: 200 calories, 100 cal from fat, 12g fat, 3g sat fat, 5mg

cholesterol

22 carbs, 2g protein------ that's a lot of carbs and calories for one measly

doughnut. but oh so delectable. and if it makes me feel like I'm not missing out

on

the joys of being alive, its definately worth it. A typical BFL meal for me ends

up being

between 250-350 calories, (without counting.......)

I guess the point is, one doughnut isn't as bad as more than one donut! and

knowledge is power! That being said, bad carbs make me crave more bad carbs. So

if

I start my day with krispy kreme its pretty much downhill from there for me as

well.

My next challenge starts today. Today I weighed in at 201. My goal is 180,

a 21 pound weight loss, sort of, I'm up a little from relaxing for four days

after the last challenge, but it's probably nonsense weight that'll be off

by Tuesday. I really didn't get off the program too much, I just didn't go

to the gym yesterday and had a big steak last night. I'm hoping to start

benching again tomorrow, it's been five weeks since my shoulder injury and I

have followed the Doc's orders to a T. I have taken 55 pounds off of my

target bench for the first 4 week cycle so I hope to come back strong. My

next competition is the first weekend in December, and I have to behave

through 5 days in Maui in November, but I'm focused and ready to go.

I totally agree with you Margeret. As far as using non-NMC registerd staff we have a quality standard that addresses this issue. The strategy is that they work within agreed competencies (A4C will also support this) and that they have adequate training, supervision and personal development opportunities. We give clients a leaflet about the health visiting team and what the role of nursery nurses is. This makes clients aware that they may recieve services from non-registered nurses but that does not mean that they get any less of a service, actually they get added value. They are also aware that these members of staff work under supervision and that the named health visitors delegate the work to them. Locally we feel that it is important that delegation is as a result of initial assessment work by a registered nurse or health visitor therefore antenatal and transfer in visits are not the responsibility of non-registered staff.

Some nursery nurses run Baby Clinics but this depends on the level of their experience, training and the client group. Clients know that the nursery nurses will defer to the HV if necessary. Most nursery nurses are more than competent to know when something needs referring for more specialist advice, it's not rocket science. Parents do it all the time without any training.

I was quite concerned about the implied implications of the discussions earlier in the week around accountability - as some people reading the discussion might think that a qualified HV or SN holding a caseload was ultimately accountable for everything that happened to any clients on that caseload whether she had had contact with them, knew them or not.

Obviously this is not true - a HV/SN can only be held accountable for work she does and this might include inappropriate delegation to a team member or failing to follow the agreed programme of visiting and not raising it as an issue if she does not have the time to do so - she cannot be accountable for missing something on a family she has not seen if the policy is to only do so many visits or to delegate work to others in the team. The issue is the competency and of course the education and training of the team member the work is delegated to. If that person is a registered nurse then he/she has their own accountability and to only do things they feel they are competent to do.

IF the team member is a nursery nurse or an HCA and the trust policy is for them to do the work as a delegate task then they as an employee and the Trust has the accountability as does the manager of the service.

Lamming in his report on Climbie was very clear about managerial accountability - and it is well worth a revisit.

I feel very strongly that accountability today within a multi agency and multi professional agenda is a much wider issue than the NMC imply in the code of conduct and I think requires much wider debate.
