It has been my goal to reduce the back strain and train my support personnel to a standard I can accept. Not easily done when only half of them speak enough English to get a concept right. " Yes boss " does not really tell me if they understand. When I ask them to actually do something, the truth becomes abundantly clear. So, frequent iteration and LOTS of actual hands-on practice seems to keep them refreshed. My scientific crew is, as others have said, my best back-up. We can communicate and many have had valuable previous medical training as first-responders or even just CPR. I have come across some new equipment bags/backpacks that are designed by , Conterra and Blackhawk that actually are designed by pack makers and designed specifically for medical response. They all incorporate excellent support systems to save our backs and manage our stuff adequately. I have no financial interest in any of these but have used much of this stuff in various capacities and found it to solve some, if not most, of the concerns voiced here about " packability " and back safety. That said, I am a huge believer in breaking up the loads to minimize the back strain. I have recently been faced with the company I have been contracted to, dictating the contents of the jump bag (although the author of this list has never been in our (offshore medics) shoes). Each time I rotate in, I remove the foley catheter kit and some of the other items from the kit that my opposite (working for contracting company) insists on having in the jump bag. Honestly, when is it appropriate to foley one's patient in the field, say 100 feet from the hospital? Appropriate inventory based on actual need and thorough assessment of the realities of the environment seem to be secondary to the dweeb inventories dictated from shore.
