Acute and chronic PA periodontitis

ACUTE : occlusal trauma is common cause

tooth tender to biting

responds to sensitivity test

widening of PDL space on xray

CHRONIC: pulp necrosis is common cause

NO responSE to sensitivity test (as the pulp is dead)

tender to biting but (mild)if any present

TENDER TO PALPATION OVER THE ROOT APEX

xray finding: minimal widending of PDL toLARGE DESTRUCTION of PA

tissue.

CONDENSING OSTEITISis the variant(increase in trabecular bone ,

xray shows

radio opaque area around the root.)

Thanks huma for wonderful explaination.could u please differentiate btw acute apical periodontitis and acute apical abcess.in both there will be pain on percussion and wd mastication.and i guess swelling could be seen in abcess but i am nt exactly sure with other pointers.your help would be appreciated.regardskanika From: humafaisal8278

Treatment range from occlusal adjustment to

RCT to extraction

(depends on pulpal condition)

ACUTE APICAL ABSCESS: is more severe inflammation

discomfort n SWELLING

SYSTEMIC INVOLVEMENT (raised temperature

and malaise)

tender to both PALPATION and percussion

Xray, well defined RADIOLUCENT AREA

could be an acute exacerbation of chronic

situation

acute exacerbation during treatment is

called :PHEONIX ABSCESS

Treatment :removal of cause asap

drainage(by opening the tooth or

incision into swelling)

antibiotic prescription(depends on

pts condition)

when acute symptoms subsides RCT or

Extraction
