Diabetes. Just say, do u suffer from a major disease such as diabetes. · Take brief social history esp. Smoking, don’t go into smoking cessation, only ask- 1. Do you smoke? 2. How many cigarettes do you smoke in a day? 3. Since how many years have you been smoking? · After seeing your teeth, X-rays and other findings, I’ve noticed that your gums and bone around the teeth are slowly losing attachment with your teeth and so the teeth are becoming loose. It seems like you have gum disease/problem (which we call periodontitis) . In this disease, you might also get problems like bad breath, bad taste, swollen and bleeding gums, there can be some pain and the teeth become loose and spaced and eventually fall out if not treated. (Diagnosis and basic explanation of disease) · It is caused by bugs which get attached on your teeth in soft whitish deposits which we call plaque. (Aetiology) · The disease can be controlled by removing these deposits by brushing regularly, in the morning and last thing at night. You might already be cleaning your teeth regularly but you may need to make some changes to the way you brush, you should use a soft brush and need to change your brush every 2 months. You’ll also need to floss everyday to clean between your teeth. To clean the plaque below the gum level and to remove the hard deposits or tartar on you teeth which collect this plaqueyou will need regular professional cleaning of teeth. (Explain methods of prevention) · Smoking reduces the response of some of your body cells which fight against this disease so smoking is a direct cause of gum diseases so in smokers this disease is more common and more severe so you have stop smoking to prevent more damage to your gums, I can give you the contact of the NHS quit smoking helpline for that. (Explain effect of smoking and advise smoking cessation) · Coming to the treatment of your problem, I’ll first need to do a thorough check of your teeth and gums and check the extent of the damage. I’ll do that by putting small instrument between your teeth and gums. Then we’ll do professional cleaning of your teeth regularly every 6 months. We might have to give you some mouthwashes like corsodyl to rinse with for a few days if we cannot control the disease by brushing alone, or I might have to refer you to a specialist. · Some of your teeth are in a very bad condition and have a poor outcome, and we might have to remove them. We can then replace these teeth with a denture, or we can use the good teeth to support a bridge. (Explain short term and long term treatment) · You will need regular check up during and after the treatment. · Do you have any questions?
