They are the ruling body in the UK for both ATLS and PHTLS. RCS Edinburgh do not have anything to do with PHTLS but have an excellent faculty of prehospital care which offers both the diploma and fellowship of immediate care. Edinburgh also certify a number of external courses. I was also wondering on what information you base your comment 'Pre Hospital Trauma Care is the trauma course all uk amb services will be adopting in the future'. The IHCD Paramedic Manual uses PHTLS as a reference source at present and many services continue to send their staff on PHTLS to supplement module H of the IHCD Paramedic Training syllabus. Furthermore attendance on a PHTLS course forms part of the DH 15 week Emergency Care Practioner course currently being used by about a dozen Trusts in the U.K. I can find no description of a PHTC course on RCS (Ed) web site. As far as I am aware the faculty offers (for those other than doctors) the Diploma in Immediate Medical Care (DipIMC) and the Pre Hospital Emergency Care Certificate (PHEC) run in conjunction with Basics (British Association of Immediate Care). The PHEC course is over three days and covers a wide spectrum of medical and trauma management and cannot be compared to a PHTLS course. Any further details on a PHTC course and how it fits into future UK paramedic training would be most welcome. ITS BAD ENOUGH TRYING TO LEARN THE STUFF IN ENGLISH WITHOUT WORKING OUT WHAT THEY ARE TALKING ABOUT IN THE FIRST PLACE. IN SOME RESPECTS IT CONTRADICTS WHAT RCS(ED) ARE SAYING ABOUT TRAUMA MANAGEMENT, FOR EXAMPLE WE WORK ON THE PRINCIPLE OF MAINTAINING A RADIAL PULSE, IF YOU LOOSE IT THEN YOU INFUSE 250ML AT A TIME TO REGAIN IT, THEREFORE REDUCING THE RISKS OF BLOWING CLOTS, PHTLS SAYS YOU RAPID INFUSE 1-2 LITRES ! A CLEAR DIFFERENCE. I SUPPOSE IT ALL DEPENDS WHERE IN THE WORLD YOU WORK AND UNDER WHOSE REMIT YOU WILL OPERATE, COVERING YOUR BACKSIDE. BOTH COURSES PRINCIPLY TEACH YOU TO WORK TO A SYSTEM IE, AcBCDEFG ETC IF YOU STICK TO A SYSTEM YOU CANT GO WRONG AND MISS THINGS, WELL THATS THE IDEA ANYWAY ! THE CHOICE IS YOURS ! -- DINGER. Guest guest Posted March 14, 2004 Report Share Posted March 14, 2004 The difference between PHTLS and PHTC is that PHTLS was originally developed by the American College of Surgeons, where-as PHTC is a private initiative developed in the UK. PHTC is an excellent course and there are some highly respected individuals in the team. PHTLS does now recognise that there is controversy over large fluid balances and advocates maintainance of BP. Several of the people who are part of the West Mids Care Team were also contributors to the fluid management consensus meeting at RCSEd and thus why this is included in PHTC. Two (maybe three) of the same team that developed the PHTC course are also responsible for FASTAid which is basic trauma management for police firearms team members. Course teaches the firearms team how to perform a focused ABC assessment and treatment. Additional skills being NP airways, O2 administration, bleeding management and use of the Ashermann seal. PHTC does NOT replace ATLS (again an ACS/RCSEng derived course) as there are many skills not taught under the former. Needle cricothyroidotomy and needle thoracentesis does not replace surgical airway and chest drain. Nor is there any inclusion of venous cutdown, pericardiocentisis or central line access amongst many of the skills taught on ATLS. The clue is in the title; Advanced Trauma Life Support. Please do the background research in a more thorough manner before making sweeping statements.
