Hi Mike-

Is he talking about a tibial tubercle transfer aka patella realignment?

That's my guess- I've had that done on both of my knees. I've got lots of info

The way I understand it is that they remove the patella and break and form

the tibia some way to take the place of the lever action of the patella in

conjunction with the patella tendon. I understand it is a fairly radical

proceedure, one that is done fairly rare on active people who have major

problems with the patella, little problems with the knee joint, and are too

young for a full knee replacement anyway. My doctor gave me two examples of

other people he did the proceedure on----one was a Navy Seal in his 20s who

destroyed his patella in an accident, and another one was a farmer about my

age (mid to late 40s).

Red Lodge is about 35 miles from us as the crow flies. I can see Lazy M

from our house. Small world.

I always thought this operation involved cutting the tibial tubercle (with the

patellar tendon attached) and moving it over to cause the kneecap to move over.

I don't have time to look it up on the Web right now, but it looks like someone

should.

You are correct, that is one form of the operation. In my case they want to

remove the patella because of advancing arthritis behind it, and they

believe the joint itself is still relatively healthy so they believe they

can save the joint. Just simply removing the patella will supposedly

relieve the pain, but would also decrease the strength of my legs by about

20% because of the mechanical leverage the patella gives to the patella

tendon. This surgery would correct the loss of strength the loss of the

patella would cause. I am trying to simplify the whole proceedure, so I

apologize for not being more clear.

I had it (tibial tubercle transfer and VMO advancement) done in 1998 when

I was 22 on my right knee and in 2000 on my left knee. Are you having the

VMO advancement done also? Personally, I would not recommend having both knees

done at once. Check out the first website- it is a huge knee message board

and a lot of people have had this surgery done recently. Search www.google.com

and www.ask.com by typing in Fulkerson osteotomy; patella realignment; high

tibial osteotomy,etc.

Thanks a lot Margaret. I think I may have mislead you somewhat. My

diagnosis is moderate arthritis under the patella causing considerable pain,

but the joint looks and operates about as normal as could be expected for

someone my age (48). The solution according to my doctor is to remove the

patella, then do a tibial tubercle transfer to help replace the lever action

of the patella on the patella tendon.

I'm not a doctor, but if your only area of damage is the underside of the

patella, then it seems to me that the tibial tubercle transfer (esp. at your age

-- no slur on people over 40 -- I'm over 60) is WAY too radical. You can have

just the kneecap and trochlea " re-treaded " . See

In my line of work, and if I want to keep doing it, it was my doctor's

opinion an artificial patella wouldn't stand up, but I haven't ruled that

route out yet. They do more of that in Europe.

I know. The FDA actually has a policy of letting European docs use a drug or

technique for 20 years before they'll approve it here.
