Only trouble is you need to have venous access as the measured drop of blood has to be a bit more specific. First saw them a couple of weekends ago when out with Staffs Amb who are at present trialing the idea. I can find out more details if you are interested..... Temp, humidity, travel, sensitivity, shelf life? There will be data available from Roche but the comparison may well have been done against traditional bio chem markers in hospital and not after it has traveled to Angola and been with customs for a while. Have you done any comparisons on your patients? Does Roche lay down strict guidelines that they will not garantee the test if the conditions have not been maintained? travel, shelf life, sensitivity? ( +2 - 8 degrees). Test kit needs to be brought to room temperature before use 15 - 20C. Roche will only Guarantee till expiry date and if kept stored at said Temp. Blood must be anticoagulated from either lithium heparin Tube or EDTA. the test also comes with a small syringe marked to 15 ul for the test it is extremely easy to use. Troponin T is first released in the blood 2 hours after Myocardial pain so may be negative if MI early. The CK/MB testing is pretty useful although it's less specific that Troponin, and I don't know if a bedside test is available. Troponin remains the " gold standard " in assessing myocardial damage. If your willing to shell out you could get a reflotron ( by Roche) about 5 grand, This has test strips for CK and CKmb ( as well as about 30 other test strips for U's & E's LFT's etc) It's available from Roche or from Porter Nash Wigmore street London. Tropinin is the gold standard for bedside testing, However.. Again I stress that with history Pain and ecg changes we do not wait for Enzymes, They are handy for that Sigh of relief after you have given the thrombolitic. CK-MB will appear within 6hrs of myocardial damage and stay raised for about 48hrs. Guest guest Posted February 5, 2002 Report Share Posted February 5, 2002 CKMB (the cardiac specific enzyme, and not CK, the more general enzyme) will show up earlier than Troponin T. CKMB will show in about 2 hours while it takes Troponin T about 6 hours. CKMB will show up earlier which is a good thing and is specific to cardiac muscle, but it's less specific than Trop T. Trop T is more sensitive, but shows up later. And on the other end of the time frames, CKMB will return to normal in about 24-36 hours while Trop T stays elevated for up to 2 weeks. So...a positive Trop T can mean an MI that is between 6 hours and 14 days old. Add CKMB and, if positive, you can narrow it down to a 6-36 hour window. A positive CKMB and a negative Trop can mean an MI that it 2-6 hours old. Obviously the EKG and clinical picture have to be considered in all of this.
