Hi all,

I was wondering if you can inform me where I can get my hands on a religious

exemption form for Illinois. My daughter is enrolled in public school preschool

b/c she receives OT and ST services. The school nurse is hounding me for

additional justification in addition to the note from her pediatrician. Thank

you.
