(Source: P.A. 85-1315; 86-1406.)Looks like the nurse is overreaching her authority. No note is required from her paediatrician; just a signed statement from you, not even notarized. Tell her, and read her the law, out loud, in front of other like minded parents and let her know she is wrong. It is about time we gang up on them. I have endured a lifetime of doctors lining up to tell me I am wrong - our turn! My daughter is enrolled in public school preschool b/c she receives OT and ST services. The school nurse is hounding me for additional justification in addition to the note from her pediatrician. Thank you. -Amy Sent from my iPhone
Guest guest
Illinois is tricky - you have to write it - I'll send info and contact people for you who can help. Don't try to do this on your own and DO NOT need a note from pediatrician regarding exemption Sheri At 11:29 AM 11/30/2011, you wrote: >Hi all, >I was wondering if you can inform me where I can get my hands on a >religious exemption form for Illinois.
