Guest guest
So, we are registering our DS, age 3, for daycare, for the first time. It will only be one day/week, the only day of the week that my work and DH's work overlap in scheduling. The daycare gave us the registration packet and of course, it asks for vaccination records. I just wrote on that line that we don't vaccinate for religious and philosophical reasons (we live in Maine). However, I'm concerned about the consent for medical treatment in case of an emergency. I don't think they'll let us register without this, but I'm concerned that he might be given a tetanus shot in an emergency situation if I didn't get to the ER in time. The authorization actually states that I give permission for any medical treatment deemed necessary by the emergency department at the nearest hospital. There is a line below this for " pertinent medical information " . If I write on that line that he is not to be vaccinated, does that create an exception to the line about " whatever treatment is deemed necessary " ? Melinda C.
