It looks pretty good, but as tempting as it is to put your opinion in or to use "vaccination," I would stick to the exact wording of the law in one sentence, and include the statute number:

I, Faeborn, parent of _____, have received information about immunizations provided by the department of health services, understand the risks and benefits of immunizations and the potential risks of non-immunization and that due to personal beliefs, I do not consent to the immunization of my son/daughter____, per A.R.S. 15-873."

My daughter here in AZ just go the form at first daycare and then at school and you just sign it.  They have them on file.  That way you don't have to explain anything or woryy about giving to much information.
