Guest guest Posted September 19, 2011 Report Share Posted September 19, 2011 It looks pretty good, but as tempting as it is to put your opinion in or to use "vaccination," I would stick to the exact wording of the law in one sentence, and include the statute number: I, Faeborn, parent of _____, have received information about immunizations provided by the department of health services, understand the risks and benefits of immunizations and the potential risks of non-immunization and that due to personal beliefs, I do not consent to the immunization of my son/daughter____, per A.R.S. 15-873." That way you don't have to explain anything or woryy about giving to much information.
