the people i know that have went to got forms didn't get hassled or anything just show bc and id sign papers etc. i know the shot records transfer so i would think blue form would to. thanks

I checked the NVIC site: (http://www.nvic.org/Vaccine-Laws/state-vaccine-requirements/florida.aspx) and under 64D-3.046, I didn't see anything specifically requiring you to hand it in more than once, but it does say "prior to admittance, attendance or transfer" which could be interpreted to attending each year.

Health forms are periodically required to be updated when booster shots or new vaccines are given, so you might be asked to hand in the exemption again with the new health form every few years. However, per FL law, you can also opt out of health assessments for religious reasons, too, and not hand either in. In high school, that might be a problem with sports, though.

At my childrens' private school, I just don't hand in the health forms more than once until they call. They've stopped calling the past few years. At their public high school, I just hand a new form in each year--I have enough battles to pick with them. But it's not inconvenient in CT the way they make it in FL where you have to go to the health dept. I'd probably refuse to in your case, and ask them to show me where in the law it says you have to.
