wording for Non Vaccinating letter requested

I need to write a letter to tell a school that I choose not to vaccinate my child for religious reasons... I live in Massachusetts and I've done this before when my son was in public school with not problems, but this September he will going to a private school in Massachusetts... so I'm requesting the wording and also if any knows that the same laws of privacy apply if your child goes to a private school in Massachusetts...thank you... Beth
