Just a statement with about wording

" the administration of immunizing agents conflicts with the

student's (name ___________________) religious tenets or

practices "

that's it

or there is a form here:

but I don't like the wording on it -

" vaccine-preventable

disease " as there is no such thing as a vaccine-preventable

disease
