Hi Winnie,

They re-evaluate every 3 years or so for their IEP. One reason is to see if they are getting services they don't need, or need services they are getting.

I don't know why the need the shot record. It is the same as the one that they got when he started that school. I understand why they may need the physical exam.

Do you think it would be OK to cross out the shot part?

I have not heard back from yet.

Besides moving from NY, I would not make any decisions without someone experienced with NY. Once you give the shot record, obviously you cannot take it back. Find out what you are absolutely required by law to give.

Are they re-evaluating whether he needs to continue at that school based on his special needs? If so, what do the shots have to do with that? Didn't they have that record from before if it is required?
