The college itself also provides you with a form right on their website: (http://www.siue.edu/healthservice/immunization/exemptions.shtml) The only thing is I don't think the law requires you to sign that you understand you have to leave campus in case of an outbreak, just that you are exempting yourself for religious reasons. But I'm not sure there's any harm in signing that (I've always used a similar one for my kids' schools). Whether you understand or not that you'll have to leave, if they are allowed to exclude you, they'll do it. I wouldn't worry about it--just thought I'd point it out. I kept mine simple. I did not tell them what my religion is I simply stated: I do not utilize vaccines due to religious beliefs. I don't think they even read it, just stapled it to the other form and said thanks. I am taking just one class, so it is technically considered less than half time, since half time is 5 hours for a graduate student.
