Pa Exemption form

Hi everyone, I live in Pennsylvania and I am enrolling my son into Kindergarten

hopefully tomorrow and I want advice for the best exemption form, I guess one

that works smoothly. My son has been to pre k and I had found and used a fill

in form online but it has you enter the age of the child, I don't want to have

to update it every year. I just want something to hand in and have that be it.

Is a handwritten form better than a typed form? Any opinions and advice is so

greatly appreciated,
