Does not say you have to tell them what religion that

is.....................just write the statement

there is nothing in the law that says you have to say which religion or

prove it.

Here is their form but they say you can't fill it out

Just submit a statement with this form with the exact wording of the

law.

ie

" I am an adherent of a religious denomination whose teachings are

opposed to immunizations "

I am from Missouri, and I just sign a form at the nurses office.

Attached, is a form I found, that you can fill out and take to the

school. I'm pretty sure you don't have to name a religious

group.

Hello,

My name is Cameron and I am the mother of two young boys, 4 years and

1 year. We currently live in Minnesota which has a philosophical

exemption, however, we will be moving to Kansas in a month for my

husband's job. They are unvaccinated and I would like to keep it that

way, however, Kansas does not have a philosophical exemption option. I am

hoping to find others here who have used a religious exemption

successfully so that I can learn from them. I'm just not sure what to do

because in Kansas they require you to identify the exact religion that

you belong to.
