Here is their form but they say you can't fill it out http://www.kdheks.gov/immunize/download/KCI_Form.pdf Just submit a statement with this form with the exact wording of the law. ie " I am an adherent of a religious denomination whose teachings are opposed to immunizations " Sheri Hello, I am from Missouri, and I just sign a form at the nurses office. Attached, is a form I found, that you can fill out and take to the school. I'm pretty sure you don't have to name a religious group. We currently live in Minnesota which has a philosophical exemption, however, we will be moving to Kansas in a month for my husband's job. They are unvaccinated and I would like to keep it that way, however, Kansas does not have a philosophical exemption option. I am hoping to find others here who have used a religious exemption successfully so that I can learn from them. I'm just not sure what to do because in Kansas they require you to identify the exact religion that you belong to.
