Trying to find that form on their site with out the exact link is awful! They

certainly don't make it easy. After I get it notarized do I send it to someone

or just carry it with me? I can't find that info any where. then do I send a

copy to the schools/ day care/ whatever? I am going to home school, but I still

have to show proof of vaccination, and I am sahm so no day care.....but I was

wondering.

Just carry it with you. I can't think of any reason you'd need it except in

another emergency room situation, to show them that you are serious enough to

have found the form and notarized it.

I don't know the laws regarding homeschooling in Iowa. But I think all schools

need to keep vaccine records, so the exemption is your vaccine record that you

have on file for your homeschool.
