Why can't I get the Alabam Religous Exemption Form

Hello,Does anyone have a copy of form ADPH-F-IMM-52 from the Alabama Dept of Public Health???? Either they have this buried in their website or they are forcing people to call/email to track them! This is ridiculous! Any help would be appreciated.Thanks!

I got an exemption for my son with absolutely no issues; I was in & out in 10

minutes. Which county are you in?

Wow! Really?! I am soo relieved to hear this. I was thinking I was going to

have a big hassle since the forms required were not readily available on their

site. What did you have to do? Did you do it recently?

I am actually not living in Alabama yet. We are moving to Maxwell AFB this

summer and I wanted to start looking for information before we got there. Not

sure if we are going to live on base or off...I have heard of some folks

mentioning Prattville being a nice area. Are you in the same county as

Prattville or the base?
