Oxycodone is good, but has a small amount of histamine release, which can cause itching. If you have a history of itching, I would avoid morphine and codeine. Fentanyl is ideal for those who experience itching because of the lack of histamine release, but it can be hard to get a doctor to prescribe. Itching is usually caused by certain opioids stimulating mast cells, which release histamine. Some opioids are notorious for this, such as: • morphine (MS Contin , MSIR , Avinza , Kadian, Oramorph , Roxanol , Kapanol) • hydromorphone (Dilaudid, Palladone) • codeine Some release histamine in moderate amounts, such as: • hydrocodone (Vicodin) Some can release histamine in smaller amounts, such as: • oxymorphone (Opana) • oxycodone (Roxicodone, OxyContin, OxyIR, etc) Histamine release is almost nonexistent in: • fentanyl (Duragesic, Actiq, Fentora, Onsolis, Sublimaze, Instanyl, Abstral) • sufentanil (Sufenta) I don't know about the histamine release for: • methadone • tramadol (Rybix ODT, Ryzolt, Ultram, Ultram ER) • tapentadol (Nucynta, Palexia) • buprenorphine (MANY brands) Nucynta does have less histamine release than tramadol, but has stronger opioid activity. I know this from experience (having been prescribed both of them). Nucynta and tramadol are not true opioids and are both very similar. They are best for people whose doctors are too stubborn to prescribe true opioids. Nucynta has moderate opioid activity, moderate norepinephrine activity, and mild serotonin activity. Nucynta is not generically available. Ultram, on the other hand, has extremely mild opioid activity, mild to moderate norepinephrine activity, and moderately strong serotonin activity. Ultram is generically available. Have you talked to your doctor about talking 'real' opioids? The butt pain really could point to piriformis syndrome. You could either google it and see if you can find stretches online or go to www.losethebackpain.com. The info on this website really helped when I was having back pain. It talks about muscle imbalances that causes tight and/or weak muscles. Doing the wrong exercises or stretches could actually cause issues. I think with fibro the muscles tend to shorten and this causes pain. I have fibro also. Guest guest Posted August 27, 2011 Report Share Posted August 27, 2011 Hi Sue, Have you considered Low Dose Naltrexone/ It has been a godsend for me for not only my muscle pain but some other non-related pains as well. I do feel stronger on it as well but I don't feel any improvement in energy as others have reported. I feel that this is a more natural, beneficial method of pain management than the heavy duty stuff or even the NSAIDS. The way that it works is that it blocks the endorphin receptors so that your body makes use of your own endorphins. It is very economical - I make my own solution using 50mg pills from a pharmacy. It is about $17 for 3 months @ 4.5 mg per day plus shipping ($25) I order many other things from them so it is not so bad. A non-med way to deal with it besides the stretches Deb mentioned is to find a massage therapist that has dealt with this before. I know others have posted about their deep independence on pain meds, and it sorrows me, knowing so many have been helped by the LDN. My integrative doc has me using 1/8 tsp or just a touch less in an 8oz glass of water to combat acidosis. Maybe using a bit more while you are experiencing pain? She has been on these heavy narcotics (pills/patches/injections) for over 10 years, maybe closer to 15. I believe (needs to be researched) that the longer you are on these OR with unresolved pain, your brain continues to produce receptors, basically trying to capture those endorphins. And LDN would take a little while to lock those receptors. Not sure that she could wait it out. It doesn't matter, though, as she is not like us. She doesn't try to get better. (nothing to do with pain management). She has other health issues and takes a fatalistic approach to all of them. And she gets sympathy from family. Different dynamic to those here on this list who want to live a full, healthy, happy life and will do what it takes to get achieve that. I do know that pain meds for some enable them to work and raise their families but I wish doctors were using this more effective method first. I slept my life away on the pain meds I was on and they were not even the heavy ones. Now, I take no narcotics, no muscle relaxers, no patches. If I have pain (usually from overdoing it), I will take one Aleve, some d-Phenylalanine, a hot bath, Aspercreme, a heating pad, etc. We all have our own path to follow. I've attended a couple webinars about its efficacy in dealing with pain.It has been strongly recommended by Teitlebaum, who I really admire. Personally, LDN has saved my life, as my pain went from totally intolerable to very minimal. I just ordered curcumin to try, but mainly it is for my husband who has colorectal cancer.
