But you WILL have to have some paperwork to show their legal exemption. And since the 'other' parent is still overseas, will it suffice if only I sign it? And..... can I even change it so it reads that it is against my religion to vaccinate against the chicken pox virus? Without having to check off any of the boxes? Oh, and do I print out the second page for them as well? Or is that just for me? Someone even mentioned that I add part of the constitution that states that I have a right to be exempt from mass vaccinations? of course unless there's an epidemic/state of emergency... If you believe in God and feel that vaccinations interfere with with His plan, you can start there. I wouldn't look at any exemption as a card to be played. Guest guest Posted December 28, 2006 Report Share Posted December 28, 2006 Hello Aldona, I don't think you have to have the form notarized (it didn't state that as a requirement), but it couldn't hurt, I suppose. I think I would amend the form to just have one line for your signature only, since your husband isn't available to sign at this time. I don't think you have the option to pick and chose which vaccines you don't want (the boxes). As I understand it's all or nothing. The second page is just for your information. I wouldn't give them that. The form states it's in accordance with MD's state law 7-403. I wouldn't include the exception to that (7-403 . My children were in junior high when I turned in their religious exemptions. They had been fully vaxed up to their kindergarten shots. No one asked any questions. It wasn't until they had all their infant/preschool shots did I realize the dangers.
