Since Sheri's away, if you can't find anything on her site, you might want to search back emails to see if you can find the people you should contact. I wouldn't re-submit my application until I'd spoken with the proper people. You don't want to say the wrong thing and make matters more difficult. Guest guest
Hi, I live in Western NY(Seneca Falls) and this is the letter I used for my daughter. Hope this helps. Tammy Seneca Family Health Center(Doctors office) May 4, 2005 To Whom it May Concern: In compliance with NYS laws, I am writing this letter on behalf of my daughter is Galka, age 6 years, who will be attending the Skoi-Yase Elementary in September, 2005. I herewith submit written statement requesting that she be excused from any immunization requirements and other invasive medical procedures because we hold sincere personal religious beliefs, which are inconsistent with these medical procedures. The practice of vaccination is contrary to our beliefs and would violate our religious principles. By declaring these convictions and presenting them in writing to you, I am complying with the immunization exemption provisions of NYS Public Health Law 2164, paragraph 9, which pertains to both public and private institutions. My daughter has been extremely healthy and we will not hold any individual or institution liable in the event she may become infected with, or ill from any infectious disease. Thank you. Tammy L. Guest guest
This is even wordier than it needs to be but rather good still. I live on Long Island, NY and have successfully used a simple form given to me by a chiropractor (written by an attorney using only the language of the law) to register my three sons in various schools including both private and public with no threatening opposition to overcome. The exemption form should be included with the reg forms with no special attention drawn to it. Like generosity, we can also be CAREFUL to a flaw by over-explaining and/or going above the chain of command. Guest guest
I was under the impression that we didn't have to write a letter but could use the exemption forms that are provided by each state. Is that true? I believe I got mine on the PAVE website (for North Carolina). Sheri B. Anita Durney <mydurney@...> wrote: This is even wordier than it needs to be but rather good still. Guest guest
Hey Sheri, When I spoke to the Director of the NYS Immunization Program in Albany back in Sept of 2002, I was told there was no official form or letter of exemption. He also told me not to anticipate any trouble from the district though sometimes they do make parents feel like they don't have a choice. He said the exemption needed to be presented for purposes of auditing only. For being the Director of the Program, he was unusually understanding and helpful to a non-vaxing mom like me (I thought). I don't know if he is still the Director, but his name is and can be reached at 518-474-1944. I think that's his direct line. The general number is 518-473-4437 if anyone living in NY is having trouble with their public school districts. He has no say in the private education arena where it's typically easier to use a religious exemption anyway. HTH Anita " Sheri B.
