According to the law, you only have to provide your written statement that you object, but not why. I used a copy of the philosophical exemption form provided by MOMs, it has the law right on it. When I signed my son up for kindegarten, they told me that I would have to sign the health department's form. I then told them to read the law and that I would not nor did I have to sign any other form and that was that. I had a feeling of relief when the pediatrician was " okay " with us not vax (okay after she gave her run down of why we SHOULD). The she came back in the room with the form and being unprepared for that- I signed it (but my gut did not feel good about it). Now what????? When I go back do I bring my own form and ask it to be added??? Guest guest Posted March 13, 2006 Report Share Posted March 13, 2006 As much as I *love* the alternative form, I would not recommend using it either if you live in a US state that only allows for religious and medical) exemptions and you plan on sending your child to school outside the home. The form makes a lot of " philosophical " statements about vaccines that would negate your religious exemption if the school board ever talked to your doctor. I've said it over and over on another messageboard...if you live in a state where you will have to file for a *religious* exemption, it is best to stick with that story from all angles. I recommend typing up a letter stating simply that " the practice of vaccinations is against my religious beliefs " without going into any detail - sign and date it (no need to have it notorized) and give THAT to your doc to put in your child's file. Guest guest Posted March 13, 2006 Report Share Posted March 13, 2006 When I wrote my exemption for Kali, I had to include a few verses from the bible neatly written in quotations and something about my family that would pertain to each of the verses. The doctor refused to take it but, the school quickly honored it. The ped. tried to talk me into getting a temporary exemption due to her gut status but, I simply told him no more. He was red in the face but, calm and talked about the CDC's so called research. I let him talk but, I still told him no more. We don't get much problem about it with him anymore. I occasssionally write stuff about what K did in church and we are going most every Sunday. We have never had any problems about it. Is this a new form that I should be seeing? Or is it something that may have been there in the past? I'm just trying to figure out if I have ever signed for non-consent of vaccination...? I know that when K was born, we had to sign for NOT giving her the Hep. shot at the hospital. That was almost 6 yrs. ago. I haven't heard anything bad about it at all yet..Hoping not to. ALWAYS use the wording of the law and it is NOT a good idea to use bible verses (for future reference). Just use the wording of hte law and in NY good to have some expert advice from some activist groups there that I have info on. Guest guest Posted March 8, 2007 Report Share Posted March 8, 2007 What about if you were to just make up a form that said something like, " Dr. X has given me information about vaccines, and I release the doctor from liability should my child come down with a disease for which the state recommends vaccinations " ? Because I do sympathize with doctors not wanting the parents to come back to them and say " you never told me my child could get meningitis if I didn't vaccinate, so pay me $4 million. " That seems like the biggest legitimate issue a doctor could have with a patient not vaccinating.
