All your dr. has to do is fax a form to hemex (they will fax a blank on request) and they will put him on their list of registered clients. Then your dr. just has to request a test kit and hemex will mail it to him with instructions. PS: You need to put 4 lbs. of dry ice in box and ship overnight to hemex.
