I keep a notebook with copies of all procedures along with a list of dates and test and results. This has helped me tremendously, saves me time and frustration. Guest guest Posted September 22, 2011 Report Share Posted September 22, 2011 I've seen Dr. Ellis in Salina and Dr. Lievens in Wichita, KS. Had botox twice w/Ellis and Lievens sent me to Dr. on, also in Wichita for my Heller Myotomy. I've had achalasia since 2003.
