Hi Stefani,

I can only offer what I might do if in your position.  Is your esophagus in a

mega condition??  Have you seen the pictures of it?  I would ask the surgeon

to

draw me a picture of where he could cut further and why.  Are they thinking of

making the myotomy longer up the esophagus?  I would have to wrap my head

around

it and ask them to explain their reasons that it may work, then make that

decision.  Will your 3rd myotomy be done Lapro?

Just curious, what was the difference between your first and second surgery?

Was the first surgery not successful?  Who is your current doctor?

I bet this is very tough on you, hang in there.

Best regards, Julee So Calif.

Subject: Third heller myotomy

Hey guys,

It's been a while since vie checked in with the group. On my last post my

doctors where leaning towards an ectomy since I've already had two myotomies.

Now they are thinking of trying a third myotomy as a last ditch effort before

the ectomy. Both my previous myotomies where open so I have a lot of scar tissue

which make having a third one more risky.

The question is, has any one from this group had three myotomies . Did it work?

And what was your experience? The surgeon said it might not work at all. Should

I bother trying it or just go ahead and push for the ectomy.

Any thoughts are welcome.

At this stage of the game, if you have not already done so, I would seek out the

'world class experts' for consultation. They are frequently sought out for more

challenging situations/most difficult cases and tend to have the most

experience with this smaller population of patients.

Julee,

Thanks fir the impute. The first hellers I had was in 93 and it didn't work at

all. The second that I had was in 95 and the cut as far as the could possibly

could go. They pretty much cut up and down the whole thing and gave me the wrap

as well. The second worked for about 15 years. Now my E has zero movement except

for the bottom sphincter. So nothing really goes down anymore. I'm not mega

esophagus yet but I'm close to end stage.I'm am a little streched out at the

bottom. surgeon says that it might not wwork at all.he just wants to go in and

see if I don't have to much scar tissue to even try. He even said if if he

finished the procedure it won't garetee how long it will last. Maybe a couple of

years. And yes he would do it lap.

I also met wit maish. And she left it up to me to either do the hellers or

the lap.. I guess I just don't know which way to go.

The muscles or scar tissue can become to tight again. We don't all scar

the same. Some have much more trouble with scarring than others. Often

these things can be dealt with by dilatation. If not, sometime surgery

can help. I don't know that there is any limit on the number of times it

can help. There is more they can do with surgery too. They may deal with

some adhesions from the old surgeries that are causing problems. They

may also do some straightening of the esophagus. There is more than just

making the myotomy. Off the top of my head I can't think of anyone that

had a myotomy and two redoes. There have been some that were told a redo

may not help but it did. There were others that the redo was not much help.

Wish I knew what you should do, but I don't.
