At what point does one need an -ectomy?

What makes a GE decide that you need an -ectomy (other than cancer)?

Size/distension of esophagus? Number and severity of spasms? Inability to

swallow even if you had a myotomy? Age of patient combined with some/all of

these problems?

Just curious if this is something I need to prepare myself for mentally.
