The theory for how it may work for acid reflux is based on the fact that the esophagus and stomach have nerves that can detect acid and cause the pressure of the LES to change. The idea is that more acid in the stomach will cause the LES to tighten and so prevent acid from refluxing. The last thing someone suffering from achalasia needs is a tighter LES. The LES in achalasia does not respond normally so it is not clear what, if anything the vinegar would do. A simple experiment would be to try vinegar before eating and then compare that with taking an antacid before eating and see if either makes eating easier or harder. Note though that acid sitting in your esophagus is a bad thing, even vinegar.
