I have NEVER been hospitalized for ANY MENTAL ILLNESS EVER.. He then goes on to state that some of my teeth looked stained but " not the usual staining from smoking but rather it looks like ACID STAINING FROM DAILY VOMITING-HE IS CONCERNED ABOUT BULEMIA!! I almost went through the ROOF when I saw this.. I have NEVER been nor will I EVER be Bulemic. As a matter of fact I have a FEAR of throwing up.. NOW I know why this man was not so anxious to help me. He thinks I " m a NUT and I'm making all of this up!! I can NOT believe what he wrote. I just fired off a letter to him and blasted him. My teeth are stained from incompete dental work in the 1990s. I had some bonding done and the teeth they bonded never got colored. I had gotten sick and stopped going to get them finished. Plus they are also stained from the years of antibiotics I've been on for Lyme Disease. You think he would have asked me? NO he jumps to some INSANE conclusion that I make myself vomit everyday? They seem to think that they know better and that you are not really having any problems and it is all in our heads. Well, all I have to say is that they are not with us 24/7/365. I would say they would have a change of attitude over what is going on with us if they could just have one really bad day with this disease. Guest guest Posted November 9, 2006 Report Share Posted November 9, 2006 When I went to be weighed before my surgery (about two weeks before) at the hospital the nurse noticed that I had lost more weight since the last time the doctor weighed me about 4 weeks previously. She asked me IF THE WEIGHT LOSS WAS INTENTIONAL now bear in mind I was about 82lb at the time DO YOU THINK IT WAS INTENTIONAL (The nurse got a very short sharp answer) When I was in hopsital a couple of days before my surgery when I was being fed by IV I had written on my notes anorexic!!!!!! I am not and never have been anorexic and would have loved to have eaten food in a decient quantity and keep it down. I heard something recently suggesting that women are still discriminated against in medical settings. I think I saw that on T.V. and they said that sometimes women's complaints (symptoms) are not taken as seriously as men's. I guess there are still some doctors out there who think of as just " hysterical women " . I was surprised by this report, but most of our recent medical experiences have been with getting help and he has pretty much been taken seriously in his complaints. He did tell me that his psychologist said that if he hadn't been diagnosed with an actual disease, he would think had anorexia. Leave it to a psychologist to say that, though. About the teeth, I'm a dental hygienist and I can tell you that long-term antibiotics won't stain your teeth once they are erupted into your mouth. That can only happen if they are taken when you're very young and the tooth enamel is still forming (before you ever see the tooth). BUT, there are many other causes for staining on teeth besides smoking or vomiting (bulemia). Some bonding materials just pick up color over time, anyway, even if you don't smoke or drink coffee. I don't think it was a GI doctor's place to make a judgement on what caused your teeth to look the way they do! It's out of his area of expertise. Good luck with your appointment tomorrow! Thanks again for all your support.. Robyn, I don't know what state you live in, but I'd bet they have a state medical board. I know Alabama does, and you can file a complaint with them. Of course a lot of them are policed by other doctors, but I'd be willing to guess, it would be effective enough to get those things off your file. I first sought medical assistance for my swallowing problems in early 1993. My general practitioner did an ultrasound of my thyroid and gall bladder, then referred me on to a gastroenterologist. The GI put me through every test in the book -- barium swallow, manometry (which he did himself and botched horribly -- the entire test consisted of two wet swallows and two dry swallows, all while I was sedated!), an endoscopy, an MRI of my chest/neck/head -- he even took a stool sample to check for blood in my stool. And of course he couldn't just CALL after each test and let me know the results -- I had to schedule an appt to come into his office, where I had to sit around waiting for an hour until he spent 60 seconds in the exam room with me to say, "This test didn't show anything, your next test will be __________ and the receptionist will schedule it for you." And then he had the nerve to charge me $90 for the privilege of having spent 60 seconds in his exalted presence. (Yes, almost 15 years later, I'm STILL bitter!) I was in my early 20s and newly divorced at the time, and although I did have insurance, I still had to pay 20% of all these tests and doctor appts, and as you all know, even 20% can add up pretty quickly. In the end, after more than six months of testing, waiting, more testing, and more waiting, he finally tells me that the tests show that there is absolutely nothing wrong with my throat and there's nothing he can do for me and I need to take tranquilizers and see a psychiatrist..... oh, and did I want to come back and see him in 90 days? I asked, "Why, do you have another boat payment due then that you need me to cover for you?" *sigh*Fast forward two years. My symptoms are so bad that I can go a full day day without even getting my own saliva to go down sometimes. I go to my ear/nose/throat specialist, who orders a barium swallow, then refers me on to a GI (a different one!) when he gets the results. This GI refers me on to the University of Michigan in Ann Arbor, and UM wants to see my records, so I get copies from the GI from he!!. Furious pretty much describes me when I saw my files, which were quite similar to Robyn's. "Patient is a hysterical young girl who seems to think food will get stuck in her throat if she tries to swallow it." EXCUSE ME??? You can ask anyone who has ever met me, even back in my teens and twenties I was NEVER considered to be "hysterical" -- I was always complimented on my maturity, in fact! I was always a straight-A student, teacher's pet, never even got a single detention ever in my life, carried a pretty heavy workload with my job even back in high school....level-headed people do not tend to get called "hysterical". "Young girl" -- I was 24 years old, for crying out loud!!!!! You can bet your butt that if I'd had a Y chromosome I wouldn't have been a "hysterical young BOY". "Seems to think food will get stuck"..... yep, that's right, I'm making this all up. It's the most common thing in the world, to have an otherwise healthy, gainfully-employed adult just make up something this off-the-wall "for attention"????? His notes follow up to say that I needed Valium and a Therapist. When I took my records to UM, it included the readouts from my manometry. I asked the GI there if, looking at my manometry (limited as it was with only 2 wet and 2 dry swallows), would he diagnose achalasia? He took one glance at the printout and said, "Absolutely, without a doubt!" Knowing more about manometry myself now, even *I* can diagnose achalasia, or at the very least a motility disorder of some sort, by looking at the swallows that are on the manometry -- the contractions are all at the exact same time, not "stair stepped" like they should be. Like you, I fired off a letter to the first GI, explaining that he was a complete quack, he completely botched my testing, and he treated me like a mentally ill moron -- had he listened to my symptoms and had he not screwed up my tests in the first place, he could have diagnosed me almost three years sooner than I was diagnosed and I wouldn't have been carrying around a ziploc baggie to spit my saliva into for so long. He did reply to me.... telling me -- get this....are you ready for it?..... telling me that "I had a diagnosis of achalasia in mind for you all along." WTF???? NOWHERE in my file does it say ANYWHERE that he even BELIEVED that I was having trouble swallowing, let alone that there was an actual reason for my symptoms. He was just back-pedaling to cover his hiney in case I sued him. I probably should have sued him, too; not for the money but to make a point, wake him up about how shabbily he treats women (girls), and put a blemish on his record for future patients to be aware of his history. Instead I've just been badmouthing him to everyone I know in town, letting them know what a misogynistic quack he is. I ran into him in an elevator at the hospital a couple years ago. I was proud of my red-headed self for just staring straight ahead. I guess I'm not as hysterical as he claimed I was, afterall, huh? In all honesty people, anorexia and bulemia is a common problem and probably more heard of to most dr's sooo don't get all furious on them. It is their job to be thorough. I initially went in saying...I feel like something is always stuck and I have to get up and leave the table cuz I feel like I am gonna choke, but I throw up. Did testing and AHEM!!!!!!! YOU HAVE A PRETTY RARE DISEASE> BINGO> I go from having no trouble to lots of trouble, to heartburn, upset stomach...feeling too full and bloated. And the other day I had tons of chest pain and bloating. I took my digestive enzyme and I felt sooooooooooooo much better in 15 minutes. Try it. It's like some of the doctors just don't know what they are doing and, since they don't know what causes achalasia, they say that we are anxious and that is how we got it. How annoying because that is not always the case! I am currently going to a psychiatrist to talk about my so called panic attacks and that I am in denial about being bulimic… I am like you afraid of vomiting and whenever I look in the mirror I go " Uch, I am so thin...I look like a skeleton " . But my doctor insist I have a desire to lose weight…I was even put on medication against bulimia and panic disorders. I quit this medication myself as I only got sick from it and lost any desire to eat even a little bit. I am going to the GI-doctor today to get result from a barium swallow x-ray. I wrote a letter about this before, but they preformed the x-ray wrongly according to a friend and doctor I know in Sweden. She is a professor and knows a lot about achalasia and have had speeches about it around the world inc. the USA. I live in Norway and they just don't know anything about achalasia here. I had ask them to check about achalasia. I think the doctor will tell me the x-ray is normal (How can he as he doesn't even know what achalasia is?!) and that they don't want to do any manometri as it is expensive. I was just on the phone with the hospital and professor in Sweden and they are prepared to talk to my doctor if I can make him call them. They are shocked about the way they treat me. It is now Friday again and I didn't get a response. So I have decided it's time for me to contact a lawyer and look into suing him for liable (spelling?).. I am so SICK and tired of doctors being able to get away with crap like This and THIS time I am not letting it go.. I can't get insurance to cover any health problems because of my journals! The insurance company denied my application for insurance based on my diffuse stomach problems and psychiatric health. Life long diffuse problems and many hospital visits are not good for an insurance application. However, I believe it will be very hard to get any statement removed from the journals. I am hoping that if I finally get a diagnosis the insurance company will have to reconsider. About my results from my doctor appointment today...NON. When I arrived the hospital they told me that they where sorry, but the doctor was home sick. I couldn't even get another appointment. I just have to wait... This happened when I was 16 more than 20 years ago. You can not imagine the funny things I hurd in the psychoterapy. The problem with this was that I did not look for help anymore. I learned by myself how to swallow (with the help of water and some diafragmatic movments). When I had problems with my voice (4 yeas ago) and the physician send me to a GI. This GI did the corret diagnose. My friens at school and university used to think that i was neurotic because of my swallowing problems. Anorexia mean loss of appetite. It has a whole host of causes from medications to stress. When it causes a fast or too large " unintentional weight loss " it becomes significant because it can indicate the person has an undiagnosed underlying disease and/or is losing too much muscle mass. If I saw a client who had lost their appetite, I would put anorexia in my notes. She could be described as anorexic in that sense of the word-someone who is suffering from a loss of appetite. The term seems to have been misapplied to you since you couldn't eat if you wanted to but it didn't imply an eating disorder. It's just a very common reason for IV feeding. And, although the answer to a question may seem obvious to you, the person asking might be required to ask as part of her job. She can't make assumptions even when the answer seems obvious. Just this morning I was asked if I had had my monthly pregnancy test. I'm 58 and had a hysterectomy 28 years ago so the answer is " doesn't apply " . They know my age at least when they call but must ask the question because it is legally mandated before they can dispense that drug. I've answered it every month for five years now and given the same answer each time. Twenty seven years ago my chart said I had had an incomplete abortion. I was upset because I would never have an abortion. When I looked it up I found out it meant a miscarriage (abortion) where the fetal body was retained which is what had happened. At that hospital an abortion was not called an abortion but a TOP or termination of pregnancy. What we would call a miscarriage is an abortion. My advice is, when reading your medical chart, make sure you understand the terms before getting upset. Read it along side a medical dictionary. The lay understanding of a term might be quite different than the medical meaning. Guest guest Posted July 14, 2008 Report Share Posted July 14, 2008 Robyn, I understand why you feel angry if looking at your chart was the first hint you had that your doctor thought you had an eating disorder. If he was wondering, he should have asked you directly about your teeth and intentional vomiting. If he never asked you that is lame. I think many of us have been accused of things like intentional vomiting, stress etc. I think it is fair for a physician to consider anorexia/bulemia as a potential diagnosis and it is also worth noting that many people with eating disorders adamantly deny that they vomit/restrict calories etc. so sometimes " indirect evidence " like stained teeth are all they have to go on. But don't you already have a diagnosis of achalasia? Was this an old entry from before you were officially diagnosed that you are just seeing now? One doctor who is also a friend of mine who works with me every day told me he thought my whole problem was anxiety. My first gastroenterologist's notes are almost funny when I read them now. He went on and on about the terrible chest pain I complained about and documented that I had been taking tums, prilosec, pepcid complete, maalox etc. and could not find relief from the severe pain. He then went on to say that he thought the pain was GERD and that I was not on any " particular regimen " to treat it and he recommended nexium twice a day..... Laughable that he did not wonder why GERD would be refractory to so many meds and also why if I could not get relief with all those together why 1 simple PPI would do it? Doctors are not perfect and they make mistakes. Not all of the mistakes are negligence but it does highlight the importance of advocating for yourself and speaking up when things just don't make sense. I hope writing the letter to set the record straight made you feel better. Are you under the care of another GI doc at this point? The same case could be made for medical records. Mistakes creep in when doctors see so many patients each day. I just looked at my medical record online this morning and was surprised to see I was diagnosed with essential hypertension two days before my surgery! I have low blood pressure and was not seen by my family doctor that day. I sent off a note to tell them to please change it. If the notes on your chart were dictated (as they often are-were they typed?), the mistake might not even be his but a clerical error. If I were you (too late I know)I would have contacted your doctor and told him you had found a glaring error in your medical records and would he please correct it promptly. Not only did it upset you but you were afraid it might work it's way into other medical records and cause you problems which would definitely then cause him problems. Then ask him to send a correction to the doctor who was to receive your medical records and to send you a copy of that letter and of the corrected medical records themselves. As for the teeth that is a valid observation. It might even be the most common explanation and doctors should always go for the most common disorder before working their way down to the less likely ones like achalasia. Ideally he should have asked you about your teeth if he was concerned enough to comment on it. A couple of questions on his part would have clarified the situation. I wouldn't worry about the bulimia comments, the doctor who treats your achalasia has seen all that before. If the old doctor thinks you are a nut, there is nothing you can do about that. It is his personal observation and he is entitled to make them. You weren't diagnosed with bulimia and that is all that is important. That is what insurance companies look at. When I carried medical records one time to a specialist the envelope was sealed. Somehow the envelope...um...became unglued (shall we say) on it's way to the doctor. Turned out to be a pretty dull read. I think it was just that clinic's practice to seal all patient records containing the doctor's personal notes. If it makes you feel any better, I was once diagnosed with " weak women syndrome " at a pain clinic. Not only is there no such thing I was later diagnosed with right heart failure and fibromyalgia. Now some doctors think the latter IAIYH (is all in your head) but the right heart failure is hard to fake. I was in severe right heart failure by the time I was diagnosed with pulmonary hypertension and that doctor gave me 18 months to 3 years to live. He was wrong. That was 12 years ago. I'm still here and now I have another rare disease. Yay for me. Well, I just got done reading his notes on me and I am livid. He said I told him I have bad panic disorder and have been hospitalized for it.. That is a LIE! I have NEVER been hospitalized for ANY MENTAL ILLNESS EVER.. He then goes on to state that some of my teeth looked stained but " not the usual staining from smoking but rather it looks like ACID STAINING FROM DAILY VOMITING-HE IS CONCERNED ABOUT BULEMIA!! I almost went through the ROOF when I saw this.. I have NEVER been nor will I EVER be Bulemic. As a matter of fact I have a FEAR of throwing up.. NOW I know why this man was not so anxious to help me. He thinks I " m a NUT and I'm making all of this up!! I can NOT believe what he wrote. I just fired off a letter to him and blasted him. My teeth are stained from incompete dental work in the 1990s. I had some bonding done and the teeth they bonded never got colored. I had gotten sick and stopped going to get them finished. Plus they are also stained from the years of antibiotics I've been on for Lyme Disease. You think he would have asked me? NO he jumps to some INSANE conclusion that I make myself vomit everyday? NO WONDER he couldn't find a reason for my swallowing trouble.. He NEVER believed me!! Has anyone else ever had this happen to them? I am so livid and upset right now. I asked him in my letter to him if he was insane. How could he even THINK someone would WANT this problem and if he knows anyone who does they need to be in a nut house not a GI's office.. You are so right to point out the human side of medical notes. You must be in the medical profession. It is interesting to be on both sides of the table at once isn't it? Dawn Re: I AM FURIOUS!! I see reminders on TV all the time warning people to review their credit histories at least once a year to correct mistakes. The same case could be made for medical records. Mistakes creep in when doctors see so many patients each day. I just looked at my medical record online this morning and was surprised to see I was diagnosed with essential hypertension two days before my surgery! I have low blood pressure and was not seen by my family doctor that day. I sent off a note to tell them to please change it. If the notes on your chart were dictated (as they often are-were they typed?), the mistake might not even be his but a clerical error. If I were you (too late I know)I would have contacted your doctor and told him you had found a glaring error in your medical records and would he please correct it promptly. Not only did it upset you but you were afraid it might work it's way into other medical records and cause you problems which would definitely then cause him problems. Then ask him to send a correction to the doctor who was to receive your medical records and to send you a copy of that letter and of the corrected medical records themselves. As for the teeth that is a valid observation. It might even be the most common explanation and doctors should always go for the most common disorder before working their way down to the less likely ones like achalasia. Ideally he should have asked you about your teeth if he was concerned enough to comment on it. A couple of questions on his part would have clarified the situation.
