Representative (insert name)

I want to urge you to support and become a co-sponsor of House Bill 3858, the

Re-Balancing Community Services Appropriation Bill.

The Chief Sponsor is Representative Sara enholtz.

It is time to move away from institutional services in Illinois. We should be

supporting the development of community services for individuals in Illinois as

they transition out of institutions. House Bill 3858 is designed for that

purpose.

HB 3858 appropriates $20,000,000 from the Department of Human Services Community

Services Fund to the Department of Human Services for grants and administrative

expenses associated with the Department's rebalancing efforts and in support of

the Department's efforts to expand home and community-based services, including

rebalancing and transition costs associated with compliance and consent decrees.

(Insert your story)

Please support and become a co-sponsor of HB 3858.

Thank you.
