Integrated Care Program Stakeholder Meeting

The next Integrated Care Program stakeholder videoconference will be held on

October 26, 2011, from 1 p.m. to 3 p.m. The department will give an update on

the program, provide follow-up on concerns raised at the August meeting, and

take questions from meeting participants. You may attend the videoconference in

person at one of two locations, or by teleconference. The meeting locations are:

401 S. Clinton, 7th Floor in Chicago, and 201 S. Grand Avenue East, 3rd floor in
