The Children with Complex Medical Needs (CCMN) meeting

is being changed from December 14, 2011, to December 15, 2011. The December 15,

2011, meeting will be held, from 1 p.m. to 3 p.m., at the following locations:

HFS Office Springfield, 3rd Floor Prescott Bloom Videoconference Room, 201 South

Grand Avenue East, Springfield

HFS Office Chicago, 7th Floor Videoconference Room, 401

South Clinton Street, Chicago

The agenda for the December 15, 2011, meeting will be

posted at a later date.

The holidays are upon us! Parents and guardians are you

in need of a short term respite in your home?

A Message about In-home Respite from Good Shepherd

Center!

Good Shepherd Center In-Home Respite Program

Through this program Good Shepherd Center

provides short-term, intermittent care for persons with a diagnosis of Autism,

Cerebral Palsy, Epilepsy or Developmental delayed in their natural, guardian or

foster family home. Our staff is sent into the family home to provide care for

the individual with special needs, as well as any siblings under the age of 18.

This allows the primary caregivers the opportunity to attend community events

with the comforting knowledge that their children are being well cared for in

their absence.

In addition to providing Respite Care for

families, this department also assists families in accessing other available

services for their child by providing referral services and to assure that the

families’ needs are best met. This department also provides CPR/First Aid

training for families at no charge.

This program is provided at no cost to the

families living in southwest suburban Cook County, northern Will Countyand the

Beverly /

Mt. Greenwood areas of Chicago. For further information

call the In-Home Respite Care Family Manager at 708-335-0020, extension 22.
