Guest guest Posted November 19, 2011 Report Share Posted November 19, 2011 Hi, I am getting ready to submit my first Annual Report of the Guardian of the Person to the 18th Judicial Circuit Court of DuPage County. I remember a post some time ago that had some good info/tips on preparing this report. I have searched the past messages, but can't seem to locate it. Can anyone recall the post or provide some input? Specifically, I was wondering: 1 - Would it would be a good idea to send the Doctor's report that we receive after our Annual Physical? 2 - If our son lives with us, what should we include under " Guardian's visits and activities " ? 3 - What info under " Other information " should we provide? 4 - Is it best to provide more or less info on the report in general? 5 - The report asks for Representative Payee Reports. Is this the same report that I send to the Social Security Administration? (Why would this be necessary if we are not guardians of the estate?) I appreciate your help and guidance.
