1--No, but in the section about medical services provided, I note the type of MD

(or DDS), frequency of visits, and month/year of most recent visit.

2--I start with " Daily interactions at home, " and that's probably redundant,

because the DuPage form already asks what the person's living arrangements

are. 

Guardian activities worth mentioning could include: met with job coach,

supervised care of your kid's assets (perhaps for the trust), negotiated with

someone to accept installment payments, assisted him in getting a state ID, did

an initial intake or update with PACT, communicated with Social Security, or

went to transition meetings with the school district.  Did you file for SSI or

SSDI, or Medicare or Medicaid in that time period?  IMHO, whatever you did that

was for your kid's benefit, that is not typical parent stuff for a kid that age

is a guardian activity.

3--Anything else you think the judge ought like to know.  Did you kid get a

job? Lose a job?  Make a change in an agency providing care?  Did you

personally cover a major expense for him?  (If so and you plan for him to repay

you in installments, say so.)  Did he get a drivers license?  A speeding

ticket?

4--Bear in mind that the court reads dozens of these a day.  They want to know

that you're taking care of your kid's needs and responsibly advocating for

him.  Give highlights that show what you've done, not a year's worth of diary

entries.

5--We have both types of guardianship.  We've been given the annual report form

(that you mention) and a form to fill out for accounting.  I've just sent both

back, without a copy of the SSA form.  If you don't have financial

guardianship, give them a call and ask them what they'd like.

Hi,

I am getting ready to submit my first Annual Report of the Guardian of the

Person to the 18th Judicial Circuit Court of DuPage County. I remember a post

some time ago that had some good info/tips on preparing this report. I have

searched the past messages, but can't seem to locate it. Can anyone recall the

post or provide some input?

Specifically, I was wondering:

1 - Would it would be a good idea to send the Doctor's report that we receive

after our Annual Physical?

2 - If our son lives with us, what should we include under " Guardian's visits

and activities " ?

3 - What info under " Other information " should we provide?

4 - Is it best to provide more or less info on the report in general?

5 - The report asks for Representative Payee Reports. Is this the same report

that I send to the Social Security Administration? (Why would this be necessary

if we are not guardians of the estate?)

I appreciate your help and guidance.

I hope someone who lives in DuPage County can help Lee...I live in Cook County

and the forms are quite different...

I researched our files and found some information from Finato about how to

file for guardianship in DuPage, but I did not see anything about how to file

the annual report.
