Guest guest Posted October 14, 2011 Report Share Posted October 14, 2011 ----- From: Terrill 'ELLEN BRONFELD' Sent: Friday, October 14, 2011 8:03 AM Subject: HFS Care Coordination Innovations Project Hi all: Ficker Terrill was kind enough to sum up the meeting on the HFS Care Coordination Innovations Project: The Illinois DFS hosted a meeting yesterday to announce that they wanted input on a new health care program for people funded by Medicaid in Illinois. The solicitation is expected to come out in January of 2010 encouraging primary care physicians, hospital and other health care providers to work together to create a care coordination entity in a specific geographic area for adults with chronic mental illness or seniors. Details about the program will be released in April of 2012. DFS also plans to releases a solicitation (call for proposals) in April of 2012 for a similar project for children with chronic health care needs funded by Medicaid. In June of 2012 there will a third proposal to coordinate care for adults who are funded with Medicaid and Medicare. At this point they are looking for input on the concepts.
