Attached you will find the slide presentation for the Care

Coordination Innovations Project meeting scheduled for tomorrow, October 13,

2011, from 10:00 a.m. to 12:30 p.m. , at the R. Center’s

Auditorium on the Concourse Level. You may wish to download your own

copy, as we will not have copies for distribution. The slide presentation is

posted on the HFS Web site at:

capacity is limited for this option, you may want to consider attending the

meeting in-person, if your schedule permits. Also, if several people from

session from one location, if logistically possible. However, if you

aren’t able to join the Webinar or come to the Center, we will

be taping the meeting and making it available on the Department of Healthcare

and Family Services Web site as soon as possible after the meeting. In

addition, following the meeting, you will have an opportunity to submit your

comments and suggestions by e-mail at hfs.carecoord@... or

on the HFS Web site at
